Thanks to Noizy for the templet to work with and to my son (Evanpp7) for his work with photoshop plus some sql.
You will need to have your sendmail / postfix set up right to use the forgot password but the rest should work just fine as long as you set up the right ips.
Was put together on Centos 6.4 with Apache 2.2.15

@2 you need to have your sendmail and or post fix set up right in order for that to work. Google has some very good tuts for that I had to use one myself to set mine up. I use postfix. My forgot password works or I would of not posted this for download with that in it. Sorry you cant get it to work, ill look for some links for you: what opp you running my server is centos 6.4.
