Login Twice Error?

Im using usa server, when i enter with an account it let me, if i log off and relog, it says: "Error!" and the same for mostly all accounts if i do the same or sometimes "ID its in use"...

 

I tryied to re enter bg_user but still same error, anyone knows?

Connector isnt setup properly (database).

its fixed thanks, user= was blank in some places in server
